Our dental plan includes:

* No monthly premiums.

* No uncertainty about whether an insurance company will contribute toward your treatment.
* No waiting period—your membership becomes effective the day you sign up.
* No preauthorization required.
* No insurance card required.
* No group or member number required.
Your membership information is maintained in your electronic patient record. Your effective date is the day you enroll. Treatment appointments remain subject to availability and your individual care needs.
What Your Enrollment Includes
* All exams at no additional charge: comprehensive, periodic, limited, and emergency exams.
* Consultations at no additional charge.
* Unlimited X-rays at no additional charge.
* Intraoral imaging at no additional charge.
* Oral cancer screening at no additional charge.
* 20% off our regular fees on all services, subject to the terms and limitations below.
X-rays and other diagnostic services are provided according to your clinical needs.
Terms and Limitations of the Plan
This plan is not dental insurance. The Peacock Dental Discount Plan cannot be used in conjunction with any other insurance coverage. It is valid only at Peacock Dental.
Memberships are not transferable between family members. One enrolled family member cannot be substituted for another.
Payment is due at the time of service. The required co-payment for any treatment received must be paid at your appointment. Any service not paid for at the time of service will be billed at our usual and customary fees.
CareCredit payments receive a reduced discount. If you choose to extend payment for treatment through CareCredit, the treatment discount is reduced to 10% due to merchant fees.
Certain injury-related treatments are excluded. This discount plan cannot be used for dental treatment following an injury when a lawsuit and outside medical, automobile, disability, or workers’ compensation insurance are involved.
